Information on Patient RegistrationRegistrationPrior to your initial office visit please call Registration at (316) 689-9888 to complete the registration process. Calls are taken between the hours of 7:30 am and 5 pm, Monday – Friday. To establish your individual file, it is important that we have accurate information including your current address, phone number, person who is financially responsible for any charges, your health insurance carrier (including policy numbers and place of employment) and other basic information. AppointmentsAppointments may be made by calling your doctor’s office or the department directly. If you would like a same-day appointment, every effort will be made to accommodate your medical needs. If you cannot keep your appointment, please cancel at least 24 hours before the appointment. When 24 hours advance notice is not possible, we would still appreciate a call so we can offer that appointment to someone else who may have an emergency. ReferralsIf your health insurance requires a referral, please contact your primary care physician to obtain one.
